Reports progress, operational issues, organizational opportunities and threats to the regional team on a monthly basis or as needed Participates actively on organizational and ad hoc committees as needed Maintains a professional facility appearance that meets patients’ expectationsĮnhances Health Center visibility through community involvement Propose, plan and direct facility expansion and enhancement projects when required. Monitors the impact of growth on the facility’s existing space plan based on projected enrollment, visits and utilization Assists in resolving provider-related issues when necessary Works in conjunction with designated Medical Director, Regional Manager and regional team to enhance provider satisfaction. Monitors appointment availability and implements changes as needed Implements effective tools to improve operational efficiencies and control costsĮnsures the coordination of provider schedules and the scheduling of patients. Monitors departmental activities and justifies variances when necessary. Prepares annual capital equipment and operating budgets for the Health Center. Promotes a team approach to delivering quality, cost-effective care to where patient satisfaction is the primary goal Provides overall direction and guidance to clinical and administrative teammatesĬommunicates organizational objectives and vision to the teamĪssists and supports teammates with the development and implementation of departmental goals, policies, procedures, budgets and reporting tools and change Knowledge and ability to document using accepted medical documentation standards Telephone triage experience a plus, with strong patient assessment skillsĪbility to provide on-site clinical treatment within scope of practiceĪbility to perform accurate telephonic triages, accurately record findings with follow-up Some healthcare industry experience within managed care and/or medical group operations environment Provide oversight in patient care evaluation, coordinate the collaboration of the Primary Care Provider and Consultants, and make suggestions to improve plans to meet patient needsĪssist with the monitoring of utilization management and make recommendations regarding effectiveness of health care resources, trending and interventionĪssist with the contestation of Part A and Part B /claims as neededĪct as the clinical resource for the non-licensed office personnel Coordinate home health and DME requests and provide recommendations to PCP Reinforce patient education regarding preventive care, dietary restrictions, medications and other therapeutic regimes. Report findings to PCP, Medical Director and Administrator Monitor inpatient, outpatient, and SNF patients and initiate patient care arrangements Maintain an active nursing appointment schedule for assisting the PCP in the monitoring of members’ healthcare needs which may include but are not limited to B/P checks, wound care needs, nebulizer treatments, and health care education ![]() Participate in health promotion activitiesĪssist the PCP in the monitoring and planning of care for members of the Complex Care Program, High Risk Program and/or Disease Management Programs Maintain accurate, legible clinical documentation within the medical records assuring compliance within all guidelines
